If you are a charity or organization looking to receive product or a donation to your organization, please click the link below and download the "Donation Request Form." You can mail, e-mail, fax, or drop off your request form at our location. All donations have to be approved prior to your event or fundraising effort.

On-site Fundraising
During certain dealer events we may allow your organization to have an on-site fundraising/PR display. For information on setting up on-site display, email: marketing@surdykemotorsports.com
Attention!
You must provide us with a flyer or publication of the event before donations are considered.
This form must be sent no later than 30 days prior to your event for review time. If you are approved
you will be notified via phone or e-mail. Applying with this form does not guarantee a donation.
Even though we wish we could help everybody’s cause or fund-raiser, Surdyke Motorsports is
limited to the amount of donations per month.
